Prior Dental Records and X-rays

Please bring any records of prior dental work to your first appointment. These medical records can be obtained by contacting your prior dental office. They are valuable in treatment planning and evaluating your dental health.

Medication List

A current and comprehensive list of all medications you are taking, including the following:

• Active prescriptions
• Over-the-counter supplements or vitamins
• History of steroid therapies or bisphosphonates

Medical History

Please provide an up-to-date medical history.  This includes any chronic or acute conditions, surgeries, and/or hospitalizations.  Please see our "Patient Forms" link above to download our Medical History form prior to your first appointment.

Please bring your insurance card to every appointment.

While we will help you to determine coverage under your insurance plan and answer questions you may have, you are responsible for knowing the details of your dental insurance and are responsible for any copays, deductibles, and all other charges applicable under your insurance plan.
